Death Claim

Step 1: Prepare Your Documents

 

  • For Natural Death
  1. Certified true copy of the Death Certificate, certified by the beneficiary
  2. Certified true copy of the insured’s National ID card, certified by the beneficiary
  3. Certified true copy of the insured’s House Registration, with the death notation stamped by the district officer, certified by the beneficiary
  4. Certified true copy of the National ID card of all beneficiaries
    • For minors without a National ID card: submit a copy of the Birth Certificate
    • For beneficiaries who are minors under 20 years old: submit a copy of the National ID card of the father/mother/legal guardian
  5. Copy of the House Registration of all beneficiaries ​
    • For beneficiaries who are minors under 20 years old: submit a copy of the House Registration of the father/mother/legal guardian​
  6. Original life insurance policy or original policy certificate
  7. Official documents supporting name/surname change, if the insured and/or beneficiary’s name does not match the policy
  8. Authorisation and consent form for the release of medical records

  • For Unnatural Death (e.g., accident, homicide, suicide, animal attack, unknown cause)

Additional documents required:

  1. Copy of the police daily report
  2. Copy of the autopsy report, certified by the police officer or forensic doctor
  3. Post‑mortem examination report (in case an autopsy was performed)
  4. Supporting or reference documents, such as newspaper or magazine articles (if any)

  • For Death by Legal Ruling
In case of being declared a missing person, prepare the following documents:
  1. Court order declaring the person as missing
  2. Final court judgment certification

3. Submit In Person at:

  • Your servicing agent
  • Claims Department, Prudential Life Assurance (Thailand) Public Company Limited
  • Any Prudential branch office nationwide 

Claim Processing Time

The Company will complete the claim assessment within 15 days from the date all required documents are received.
In cases where additional investigation is necessary, the assessment will be completed within 90 days.

Identity Verification Requirement (per Anti‑Money Laundering Office regulations)
According to the notification from the Anti‑Money Laundering Office (AMLO), if the claim payment to any beneficiary is THB 100,000 or more, the beneficiary is required to complete identity verification by either:

  • Clicking here: https://services.prudential.co.th/ekyc-ui/, or

  • Submitting:
    • A recent color photograph (front-facing, clearly showing the face, taken within the last 6 months), and
    • A clear color photograph of the National ID card
  • These documents can be sent to the Company by postal mail.
  • For beneficiaries who are minors and do not have a National ID card, please provide identification details and verification of the father, mother, or legal guardian.
